Fields click karke judte hain (drag-drop nahi); ↑ ↓ se order badlo. Form staff screen par bharta hai ya khaali form print karke kaagaz par bharwaya jaata hai; bhara form patient record me save hota hai. Bhare forms ka print (jawab ke saath) abhi nahi nikalta — screen par "View" hota hai. Sab clinic ke bhare forms: Custom Forms → "Filled forms" tab (form/patient se filter, View, delete owner/admin, "Fill for a patient").
Apna custom form kaise banaye (Form Builder)?
Setup → Settings → Clinical → Custom Forms → "New Form" → naam + category → left palette se fields click karke jodo (text, dropdown, checkbox, signature, table…) → "Save". Patient chart → Forms → Custom Forms se "Fill" ya "Print" karo.
