Validity rules:
- Entry Level Dental: 2 saal. Renewal expiry se 6 mahine pehle apply karo; agar expiry se 3 mahine pehle tak apply NAHI kiya to NABH maan leta hai ki interest nahi hai — certificate expiry date pe laps, continuity tooti, fir FRESH application (poora process dobara). Renewal me poori application dobara bharni hoti hai; chair count badla ho to nayi category ki fee lagti hai.
- Full DHSP: 4 saal ka cycle. Year 2 me surveillance assessment hota hai (24 mahine baad). Renewal/re-accreditation bhi expiry se 6 mahine pehle. Surveillance me ≥80% 'achievement' level chahiye; renewal me ≥80% 'excellence' level aur KOI element ≤2 nahi.
- NABH surprise/un-announced visit bhi kar sakta hai (media me adverse report aane pe bhi), aur scope/address/key-person change pe focus assessment.
- Cancellation ho sakti hai: fees na bharna, regulatory action, serious patient-safety issue.
