NABH ke liye kaunse license documents pehle se chahiye?

Portal pe upload karne padte hain: clinic registration (Clinical Establishment/Shops Act/municipal — koi ek), PCB ki BMW authorization + waste vendor MOU, AERB eLORA license har X-ray machine ka, Fire NOC (ya third-party fire audit), electrical audit (1 saal ke andar), lab MOU, emergency hospital MOU, 6 mahine ki water testing reports.

Official legal tracker (Entry Level Dental standard, Section 2) — application se PEHLE ye sab active aur scanned ready rakho:

  1. Clinic registration — koi EK chalega: Clinical Establishment Act, Shops & Establishments Act, municipality/panchayat registration (MSME/Udyam, Labour Ministry bhi accepted).
  2. State Pollution Control Board se BMW AUTHORIZATION (sirf vendor registration kaafi NAHI) + authorized Common Bio-Medical Waste Treatment Facility (CBWTF) ke saath MOU. Annual BMW report har saal 30 June tak SPCB ko.
  3. AERB license/registration eLORA portal se — HAR X-ray machine ka alag (IOPA, OPG, CBCT sab track hote hain). Radiology outsourced hai to us center ke saath MOU kaafi hai. AERB license ~5 saal valid — renewal miss mat karo.
  4. Fire NOC — ya agar building exempt hai (12m se neeche) to NABH fire-safety guidelines ke hisab se THIRD-PARTY fire audit report. Mall/complex ka common Fire NOC clinic ko cover karta hai.
  5. Facility ka third-party ELECTRICAL audit (pichle 1 saal me).
  6. Outsourced dental lab ke saath MOU.
  7. Kisi hospital ke saath emergency/ambulance MOU.
  8. Pichle 6 mahine ki water-testing reports.
