Har daant ke liye Mobility (M0–M3), Furcation (F0–F3) aur X = missing daant. Upar 'Show:' tick se PD / REC / BOP / SUP / PLAQUE rows on-off (shuru me PD + BOP). Exam date, Classification (healthy / gingivitis / stage I–IV) aur Grade (A/B/C) bhi chuno. Jo daant nahi hai use X zaroor karo, warna uski khaali site 0mm gin kar BOP % / Avg PD kam aate hain. Save ke baad exam 'Periodontal Charts' list me aata hai; row click karke purana chart dekh sakte ho (edit / delete nahi hota). Mobile: patient → More → Perio → "6-Site Chart" (PD, BoP, PI aur missing; recession / mobility / furcation nahi). Treatment → Clinical → Specialty → Perio Chart page par bhi exam ho sakta hai, par wahan sirf PD aur BOP hain.

Patient chart kholo → left sidebar 'Sections' me ⋯ More → 'Perio'. Wahan 6-site WHO periodontal chart hai: "New Exam" → har daant ke MB/B/DB/ML/L/DL site par PD likho, BOP tick karo → "Save Chart". BOP %, Plaque %, Avg PD aur >4mm / >6mm sites apne aap nikalte hain.
